As a CareCentrix® Intake Manager with Gentiva, you will:



*Manage all intake processes/operations to ensure a smooth transition from payers/referral sources to staffing/care management.

*Ensure that intake services are consistent with CareCentrix policies, service standards and applicable laws for assigned region.

*Ensure that all incoming telephone calls from referral sources/potential clients are promptly/effectively responded to and ensures that payer benefits information is secured/maintained for the geographic coverage area.

*Determine CareCentrix's ability to provide requested services and identify alternative resources for callers when CareCentrix solutions are not available.

*Perform initial evaluations of referrals' appropriateness for CareCentrix services and pass all intake information to the Care Management Manager.

*Establish and maintain ongoing relationships with referral sources. Make recommendations and consult with the Director regarding the acceptance of referrals that do not meet the Gentiva/CareCentrix guidelines.

*Ensure that all potential/primary payers are accurately identified. Document demographic, clinical and payer information on all referrals and determine coverage availability for third party payers.



Qualifications



*Bachelor's Degree or Diploma in Nursing or the equivalent

*Current, unrestricted R.N. licensure in the state(s) of practice plus

*A minimum of five years nursing experience preferably in an HMO environment

*Two years in a managerial role

*Thorough knowledge of healthcare delivery/managed care regulations, case management, reengineering processes, contract terms/stipulations, capitation requirements and governmental home health agency regulations

*Excellent negotiation, communication and decision making skills
